How Do I.... Register With The Practice?

1. Check with our reception staff that you are living within the practice area.

2. If so, you may collect the registration forms from reception and the registration process will be explained to you. (Note: The registration process will include a new patient medical with the nurse.) You will be asked to provide identification.

3. When you register at Sixways Clinic you are registering with our practice. We do, however, encourage you to see the same doctor wherever possible and will indicate this on your registration card.
